Once a patient understands and approves a treatment plan, the navigator works with the cancer care team to make sure members are aware of the plan of care that has been approved. Throughout treatment, the nurse navigator continues to communicate with the patient and team members as needed. If a patient is experiencing side effects, a nurse navigator can alert the care team and explore options to reduce the effects, such as switching or adding medications.

Referrals

A nurse navigator will identify the need for a referral to another healthcare professional. The navigator might set up an appointment with a dietician, social worker or rehab facility.
